Definition 1 (Concise / Popular Use): When a medical institution, healthcare provider, or system withholds, omits, or deliberately excludes critical medical records, information, or care, leading to harm, neglect, or denial of rights. A modern form of medical apartheid where the damage isn’t just from what’s done, but from what’s left undone. Definition 2 (Formal / Academic Use): A systemic pattern where healthcare professionals fail to document, acknowledge, or accurately represent the concerns, symptoms, and advocacy of patients — disproportionately harming vulnerable populations such as Black American (Negro) (Freedmen). Also, disproportionately harming vulnerable populations such as United States military active-duty service members and veterans, who already face the highest suicide rate in the United States of America.
