A high D is the second octave of the open D chord on a guitar, so it occurs at the top end of the blues solo region in the key the guitar is tuned to. It's also someone who's twice as tall as the next tallest person so their "D" is high in the air, or at head height. Instead of shaking their hand, they make you suckit.
