Short/slang for "You Heard Me?". Typically it's used rhetorically & a response is not warranted. Any of the following phrases could be substitued for it depending on the context and/or the user's tone, "That's crazy, right?", "Know what I'm saying?" or "No bullshit!". It's commonly used in urban areas, most frequently by those influenced by hip-hop/rap culture. Big in Chiraq and the Midwest right now. It's pronounced quickly, as one word w/two syllables: Yerd-me.
